Job Title: Case Manager

The Case Manager ensures follow-up with a client’s collaterals and works in conjunction with the clinical team to create discharge plans.

Essential Duties and Responsibilities

  • Utilizes FRN endorsed client-centered therapeutic interventions and rapport-building practices, including motivational interviewing techniques, to non-confrontationally enhance consumer insight in an effort to develop a comprehensive discharge plan.
  • Documents referrals, family, employer and other contacts (during treatment and at discharge) according to periodicity and content requirements.
  • Maintains awareness of client needs upon entry, utilizes such information to develop appropriate discharge plans, and implements plans accordingly.
  • Coordinates comprehensive discharge plan with needed resources in the community to ensure seamless community reintegration.
  • Coordinates with treatment team regarding daily census.
  • Oversees all aspects of discharge planning and coordination.
  • Communicates referrals as necessary.

Education and Experience

  • Bachelor’s degree in social work, counseling, psychology or related field required or commensurate experience.
  • CADC certification in addictions preferred.
  • Minimum of 5-10 years of experience in social service setting or treatment facility strongly preferred.
